We request that payment for services, the deductible and anticipated co-pay or percentage due payments are made on date of visit. We accept Cash, Mastercard, Visa and Discover. For those patients with private health care plans, we will, as a courtesy, submit patient claims. Your insurance company will reimburse you directly for the benefits to which you are entitled. Please contact your insurance company for reimbursement information.
